What would you include in Shenmue 3 so it's more realistic?

Shenmue 2 added extra realism by (among other things) allowing you to choose questions to ask, something I'm forever grateful for. What could be added to Shenmue 3 to make it even more realistic? Could be minor details like more dialogue for each NPC, or bigger details like characters not always wearing the same clothes.

Eating regional foods.

Maybe cooking and eating your recently caught fish.

If the game goes "survival mode" there could be some interesting stuff.

Getting clothes dirty/wet.

Injuries / stamina after fights/training.

But yeah, the most important is npc behavior/AI.

Introduce the requirement for Ryo to drink and feed himself in order to keep a good stamina.

I don't think they're going to focus on additional survival features. Ryo will probably go through the game without eating or using the bathroom. If they focus on something, it will be maximizing the dialog and authenticity of the landscapes, cities and people.
